STEP 1: FILE

BPLO: Verify and Validate application form including requirements.

BACKROOM OPERATIONS

MEO/MENRO/MHO/BFP: Verify records and endorse the application as to approve/disapprove. Prepare clearance.

 BPLO/MTO: Assess taxes, fees and other charges and issue Tax Order of Payment.

 BFP: Issue Order of Payment for FSIC.

STEP 2: PAY

MTOIssue Official Receipt after payment has been received.

BACKROOM OPERATIONS

BFP: Issue Official Receipt for FSIC.

BPLO: Release the approved Mayor’s Permit, Business Plate and other Clearances..

1. DTI/SEC/CDA REGISTRATION
2. BARANGAY BUSINESS CLEARANCE
3.OCCUPANCY PERMIT (Bagong Tayong Gusali)
4. CONTRACT OF LEASE (Kung Umuupa)

RENEWAL
1. BASIS FOR COMPUTING TAXES (Gaya ng ITR, BIR Quarterly/Monthly, Financial Statement, etc.)
2. BARANGAY BUSINESS CLEARANCE

ADDITIONAL REQUIREMENTS
1. NOTARIZED AUTHORIZATION LETTER (If by representative)
2. PAYMENT FOR REAL PROPERTY TAX
3. LIST OF WORKERS/EMPLOYEES
4. COMMUNITY TAX CERTIFICATE
